Arizona SLP License Requirements:
Candidates must meet Arizona's requirements to provide school-based SLP services, including:
Arizona Department of Health Services SLP license
Arizona Department of Education PreK–12 SLP Certificate
Master's degree in Speech-Language Pathology or Communication Disorders
Required clinical experience/clock hours
Passing national examination or valid ASHA CCC-SLP
Valid Arizona DPS IVP Fingerprint Clearance Card
Arizona DOE specifically states that school-based SLPs must hold both the PreK–12 SLP certificate and an Arizona Department of Health Services SLP license.
