Speech Language Pathologist (SLP) in Lincoln, CA
Provide expert assessment, diagnosis, and treatment for speech, language, cognitive-communication, and swallowing disorders in a medically complex geriatric population within a subacute/long-term skilled nursing facility environment. This contract position requires collaboration with an interdisciplinary team to ensure quality patient outcomes and regulatory compliance.
Key Responsibilities:
- Conduct comprehensive evaluations within 24 hours of physician referral to determine functional therapy diagnoses and prognoses.
- Develop and implement individualized, evidence-based treatment plans with clear, measurable goals.
- Deliver direct therapy addressing communication and swallowing disorders, including compensatory strategies and cognitive-communication interventions.
- Maintain thorough and timely documentation including evaluation reports, progress notes, physician updates, discharge summaries, and billing records.
- Educate residents, families, and nursing staff on communication strategies, safe swallowing techniques, and home exercise programs.
- Coordinate care with nurses, physicians, therapists, dietary, and social services to optimize outcomes and discharge planning.
- Participate in quality improvement efforts and ensure compliance with Medicare/Medicaid documentation standards.
- Provide supervision for SLPAs or therapy aides in accordance with California regulations when applicable.
Qualifications:
- Master’s degree in Speech-Language Pathology from an accredited program.
- Current California SLP license or temporary RPE license in process.
- Certificate of Clinical Competence (CCC-SLP) preferred.
- Experience in skilled nursing or geriatric rehabilitation settings preferred.
- Proficient in clinical assessment, treatment planning, and interdisciplinary communication.
- Knowledge of Medicare/Medicaid billing, PDPM, and California regulatory requirements.
- Current BLS/CPR certification required.
