Speech Language Pathology (SLP) | Lincoln, California
This opportunity involves providing specialized speech-language pathology services in a skilled nursing facility setting, focusing on geriatric patients with medically complex needs. The role centers on assessing, diagnosing, and treating communication and swallowing disorders in a subacute and long-term care environment. Collaboration with an interdisciplinary team is essential to support patient outcomes and comply with regulatory standards.
Key Responsibilities • Conduct comprehensive speech, language, cognitive-communication, and swallowing evaluations within 24 hours of referral, establishing functional diagnoses and prognoses. • Develop and implement individualized, evidence-based treatment plans with clear, measurable goals, adjusting therapies based on patient progress. • Deliver direct therapy addressing communication and swallowing challenges, including compensatory techniques, cognitive interventions, and group sessions as appropriate. • Maintain accurate and timely documentation, including evaluation reports, daily treatment notes, progress updates, physician reports, discharge summaries, and billing records. • Educate residents, families, and clinical staff on effective communication strategies and safe swallowing practices. • Coordinate care with nursing, physicians, PT/OT, dietary, and social work professionals to optimize patient outcomes and aid discharge planning. • Participate in quality improvement initiatives and ensure compliance with Medicare/Medicaid regulations, including PDPM and Title 22 requirements. • When applicable, supervise SLPAs or therapy aides per relevant California regulations, providing direct supervision and meeting CEU requirements.
Qualifications and Experience • Master’s degree in Speech-Language Pathology from an accredited institution. • Active California SLP license; temporary Resident Provisional Evaluation (RPE) license accepted if in process. • Certificate of Clinical Competence (CCC-SLP) from ASHA preferred. • Experience in skilled nursing or geriatric rehabilitation settings is highly desirable. • Proficient clinical assessment and treatment planning skills with strong organizational and interpersonal communication abilities. • Working knowledge of Medicare/Medicaid billing, PDPM requirements, and California healthcare regulations. • Current BLS/CPR certification required.
