Provide specialized speech-language pathology services within a subacute and long-term skilled nursing environment, focusing on a geriatric, medically complex population. Utilize your expertise to assess, diagnose, and treat speech, cognitive-communication, and swallowing disorders while collaborating with an interdisciplinary team to improve patient outcomes and ensure regulatory compliance.
Location: Roseville, CA
Key Responsibilities: • Conduct comprehensive speech, language, cognition, and swallowing evaluations within 24 hours of physician referral; develop functional therapy diagnoses and prognoses. • Design and implement individualized, evidence-based treatment plans; set clear, measurable goals and adjust therapy based on patient progress. • Deliver direct therapy including compensatory strategies, cognitive-communication interventions, and group sessions as appropriate. • Complete timely and accurate documentation including evaluations, daily treatment notes, progress updates, discharge summaries, and billing records. • Educate patients, families, and nursing staff on communication strategies and safe swallowing techniques. • Collaborate closely with nurses, physicians, PT/OT, dietary, and social work teams to optimize care and support discharge planning. • Participate in quality improvement initiatives and maintain compliance with Medicare/Medicaid regulations. • If supervising SLPAs or therapy aides, ensure adherence to California’s regulatory supervision requirements.
Qualifications: • Master’s degree in Speech-Language Pathology from an accredited program. • Current California SLP license or temporary RPE license in process. • Certificate of Clinical Competence (CCC-SLP) preferred. • Experience in skilled nursing or geriatric rehabilitation settings highly preferred. • Proficiency in clinical assessment, treatment planning, and Medicare/Medicaid billing regulations (PDPM, Title 22, CMS). • Current BLS/CPR certification. • Strong communication, organization, and interpersonal skills.
