Speech Therapist in Rocklin, CA
This position offers an opportunity to assess, diagnose, and treat speech, language, cognitive-communication, and swallowing disorders within a geriatric, medically complex population in a subacute and long-term skilled nursing environment. You will collaborate with an interdisciplinary team to support quality patient outcomes and ensure regulatory compliance.
Responsibilities include:
- Conducting comprehensive evaluations within 24 hours of physician referral to assess speech, language, cognition, and swallowing function.
- Developing individualized, evidence-based treatment plans with clear, measurable goals.
- Providing direct therapy addressing communication and swallowing disorders, including compensatory strategies and group therapy when appropriate.
- Documenting timely and accurate evaluation reports, treatment notes, progress updates, physician reports, discharge summaries, and billing documentation.
- Educating residents, families, and nursing staff on communication strategies and safe swallowing techniques.
- Coordinating care with nurses, physicians, PT/OT, dietary, and social work staff to optimize patient outcomes and support discharge planning.
- Participating in quality improvement initiatives and ensuring compliance with Medicare/Medicaid (PDPM, Title 22, CMS) standards.
- Supervising SLPAs or therapy aides according to California regulations when applicable.
Qualifications:
- Master’s degree in Speech-Language Pathology from an accredited institution.
- Current California SLP license or temporary RPE license in process.
- Certificate of Clinical Competence (CCC-SLP) from ASHA preferred.
- Experience in skilled nursing, geriatric, or rehabilitation settings preferred.
- Strong clinical assessment, treatment planning, communication, and organizational skills.
- Knowledge of Medicare/Medicaid billing, PDPM requirements, and relevant California regulations.
- Current BLS/CPR certification.
The work environment involves managing subacute rehabilitation stays (up to ~100 days under Medicare Part A) and long-term care residents. Therapy intensity may reach about three hours per day based on tolerance, with productivity expectations around 80-90% of available work time billed to therapy. Flexibility is needed to manage fluctuating workloads and foster collaborative relationships with nursing and support staff.
