Speech Language Pathologist (SLP) in Canastota, NY

Responsibilities
- Conduct comprehensive speech-language assessments and develop individualized treatment plans.
- Provide therapy services to patients in a long-term care setting.
- Collaborate effectively with interdisciplinary teams including medical and nursing staff.
- Utilize electronic medical record systems, including Epic/EMR, to document patient progress accurately.
- Apply knowledge of the PDPM reimbursement system in clinical documentation.
- Maintain excellent communication with patients, families, and healthcare teams to support recovery goals.
Qualifications / Desired Experience
- Graduate of an accredited college/university with a degree in Speech Language Pathology.
- Current New York State license as a Speech-Language Pathologist.
- Eligibility for or holding the Certificate of Clinical Competence (CCC) from ASHA.
- Proven experience in long-term care settings, with demonstrated assessment and therapeutic skills.
- Proficiency in Microsoft Office and Epic/EMR software.
- Strong communication skills and ability to make accurate patient prognoses.
Schedule
- Shift hours: 7:00 AM to 3:30 PM
