Speech Therapy - Santa Clara, California
A rewarding opportunity is available for a Speech-Language Pathologist to provide comprehensive therapy services in a skilled nursing environment. The facility includes a 256-bed Skilled Nursing Facility with three skilled nursing stations, a sub-acute unit, and a locked memory care unit, delivering a wide range of patient needs in a supportive setting.
Key Responsibilities:
- Provide individualized speech therapy evaluations and treatment plans for skilled nursing residents
- Collaborate closely with a multidisciplinary team including physical and occupational therapists, as well as nursing staff
- Document therapy progress using the facility’s EMR systems, Nethealth and PCC, with training available for those unfamiliar
- Support residents on both skilled nursing and sub-acute units to improve communication, swallowing, and cognitive functions
- Participate in team meetings to coordinate therapeutic interventions and optimize patient outcomes
Desired Qualifications:
- Valid Speech-Language Pathology license
- Experience or interest in skilled nursing therapy services preferred; training in EMR systems will be provided
- Strong communication and interpersonal skills for effective teamwork and resident interaction
- Ability to work full-time, with the option to split time between two locations within the organization
