Speech-Language Pathologist (SLP) Home Health

Location: Lake Zurich, IL
Job Type: Full-Time, 13-Week Contract (with potential extension)
Schedule: Monday–Friday, 8:00 AM – 5:00 PM
Pay Range: $40–$47 per hour

Overview:

Sunbelt Staffing is seeking a licensed and experienced Speech-Language Pathologist (SLP) for a 13-week contract opportunity in a Home Health setting in Lake Zurich, Illinois. This role offers the chance to provide in-home speech therapy services focused on restoring patient communication, swallowing, and cognitive functions. The ideal candidate is dedicated, detail-oriented, and passionate about patient care.

Key Responsibilities:

• Conduct comprehensive patient evaluations and develop individualized treatment plans
• Provide speech therapy interventions to improve patient outcomes
• Monitor and document patient progress, adjusting plans as necessary
• Collaborate with caregivers and interdisciplinary team members
• Travel to patient homes within the local area (mileage reimbursed)
• Maintain accurate, timely documentation according to agency and regulatory standards

Qualifications:

• Master's degree in Speech-Language Pathology from an accredited program
• Active SLP license in the state of Illinois
• Certificate of Clinical Competence (CCC-SLP) from ASHA required (or eligible)
• Valid driver’s license and reliable transportation
• Basic Life Support (BLS) certification required
• Prior Home Health experience preferred
• OASIS experience preferred
• Familiarity with Home Care Home Base (HCHB) EMR preferred
• Strong clinical skills and excellent communication abilities
